When requesting a canine/feline stripdown you will be asked to sign one or the other of the following legal forms. While these forms describe the worst case scenarios about what can occur during and after a strip, it is important that you understand what a strip really is and means. Please read the below very carefully. After signing the release you will receive a copy of the form before the groom proceeds.
MATTED DOG/CAT STRIPPING RELEASE FORM: Matted coats can cause a variety of skin & health problems. Matted fur does not allow for air circulation to the skin, causing hot spots, bacterial and fungal infections. Fleas, ticks, maggots and other parasites may be lurking in the coat causing further skin infections. Matted fur also pulls and binds, causing pain to your pet when they move or lay on the mats. The skin underneath is usually raw and inflamed. Matted coats will not dry properly and can lead to rotting fur and skin.
Due to matting, pets' hair may need to be shaved extremely short to the skin and may show signs of skin irritation and sores due to matting, wet undercoat and dirty coat. Pets that have not been groomed often enough usually result in long, stressful, or painful grooming sessions. Some animals never recover from that experience. There is a strong chance that your pet's skin will become more irritated during the clipping process. Due to the mats being tight against the skin, the only way of removing them is to use a short blade to clip between skin and mats He/she may be nicked and/or cut, and exhibit signs of distress for some time (up to a couple of weeks) after the strip, because I (Karen Boehm) have to work so closely to the skin to remove the coat. I am very careful and will take every precaution available to me not to inflict harm, but the possibility of injury is there, and you need to be aware of this and give me permission to proceed.
It is the goal of Dirty Hairy Mobile Pet Grooming, LLC to make your pet's grooming experience as stress free as possible, however, due to the fur condition of ________________________, Dirty Hairy Mobile Pet Grooming, LLC has determined that the only humane thing to do is a total shave down, because it is unfeasible to de-mat/brush out and would cause excessive pain and suffering to attempt to do so at this time. I, as owner, having read and understood the above terms, agree this is the only option.
I hereby release Dirty Hairy Mobile Pet Grooming, LLC/Karen Boehm from any responsibility for injuries incurred by the above-mentioned process and any and all medical problems that may be uncovered, and/or occur during or after the stripping process.
Should my pet need veterinary care during or after the process, I, as the owner, agree to pay any and all veterinary fees for the my above-mentioned pet's care. I have received a copy for my records.

SUMMER SHAVEDOWN RELEASE FORM: A summer shave down is defined as the pet's hair being shaved off very close to the skin (1/8th to 1/4 inch). I understand that the process of shaving down does carry some risk and may incur nicks and/or cuts, and other skin irritations, especially in the instance where any mats are discovered in the fur and close to the skin. In some cases a pet may also exhibit behavioral changes for some time after the shave down. I do not hold Dirty Hairy Mobile Pet Grooming, LLC/Karen Boehm responsible for any injury that may occur as the result of a summer and/or full shave down or any and all medical problems that may be uncovered, and/or occur during or after the summer shavedown process. I have also been advised by Ms. Boehm that my pet runs a risk of sunburn until such time as his/her coat has grown further out and understand that I should not allow my pet long exposure to full sun during that time due to risk of melanoma.
As __________________________'s owner I, the undersigned, have requested a full summer clipdown for my dog/s. I understand this is a very short clip which will also remove any matting that may be present and in consideration of the grooming services of Dirty Hairy Mobile Pet Grooming, LLC/Karen Boehm, agree to hold Dirty Hairy Mobile Pet Grooming, LLC/Karen Boehm harmless from any damages.
I also indemnify Dirty Hairy Mobile Pet Grooming, LLC/Karen Boehm from any loss or claims arising from any known or unknown preexisting condition of my pet(s) including but not limited to advanced age, extreme nervousness, neurosis, tumors, illness, previous injury, skin or coat conditions and medical conditions.
I have read and agree to these policies and by signing below do hereby give my permission to Dirty Hairy Mobile Pet Grooming, LLC/Karen Boehm to proceed with the strip. I have also received a copy for my records.
